Responsibilities

• Provide direct nursing care under supervision of the Registered Nurse and interdisciplinary care team.
• Administer prescribed medications and treatments in accordance with facility policies and procedures.
• Monitor resident conditions and promptly report changes or concerns to the RN.
• Perform routine clinical tasks including vital signs, wound care, catheter care, and specimen collection.
• Assist with admissions, transfers, and discharges as needed.
• Reinforce resident and family education provided by the RN and care team.
• Maintain accurate, timely, and complete clinical documentation.
• Support implementation of individualized care plans.
• Promote resident safety, comfort, and dignity at all times.
