Job summary

Sage Specialty Hospital (LTAC) in Opelousas, LA is hiring Licensed Practical Nurses (LPNs) for full-time and PRN positions. • Must be a graduate of an accredited School of Nursing.
• Current and active Licensed Practical Nurse (LPN) Louisiana license.
• Current CPR certification.
• Must be able to work a flexible schedule including 12‑hour shifts.
• Have dependable transportation.

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other team members to coordinate optimal care for the patient/family.
• Document all services provided and submit them on appropriate forms and Report changes in patient condition and need to RN.
