Responsibilities

• Evaluate and deliver care including medication administration and ADL assistance according to physician orders and plan of care.
• Conduct regular rounds to monitor resident activity and ensure quality care per standards and facility policies.
• Communicate with residents, family members, physicians and supervisor regarding resident status.
• Maintain up-to-date medical records during tour of duty on assigned unit.
• Other tasks as required.

Licensed Practical Nurse ( LPN ) duties include but are not limited to:
