We expect you to supervise CNAs, participate in evaluation processes, and contribute to Falls and Pressure Sore Programs, ensuring high-quality, person-centered care for residents.

Qualifications

• Must possess a current, unencumbered, active LPN license
• Experience in Skilled Nursing/Rehabilitation facilities preferred
• Be compassionate and dedicated to high-quality patient care

Responsibilities

• Provide direct patient care and implement individualized treatment plans under supervision
• Assist residents with activities of daily living

• Oversee Falls Program and Pressure Sore Program

As a LPN, you will provide direct patient care to our residents and patients, ensuring they receive exceptional, person-centered care. About the Role

As a Licensed Practical Nurse (LPN), you will be responsible for providing direct patient care to our residents and patients, assisting them with activities of daily living, and implementing individualized treatment plans under the direction of a professional nurse. Your responsibilities may include:

• Assisting in the implementation of individualized treatment plans for each assigned resident
• Supervising CNAs and participating in their evaluation
• Overseeing Falls Program and Pressure Sore Program
• Providing nursing services
• Conducting and documenting a thorough evaluation of each resident’s medical status upon admission and throughout their course of treatment
• Other duties as assigned

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN
• Experience in Skilled Nursing/Rehabilitation facilities preferred
• You must be qualified, compassionate, and dedicated to a job well done
