Aviata at South Daytona is seeking a Licensed Practical Nurse (LPN) to provide direct patient care in a skilled nursing and rehabilitation setting. You will follow individualized care plans, document progress, and assist in daily living activities under the supervision of a registered nurse.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.

Responsibilities

• Provide direct patient care to residents and patients of the health and rehabilitation center (skilled nursing home), assisting them in activities of daily living under the direction and supervision of a professional nurse.
• Assist in the implementation of individualized treatment plans for each assigned resident.
• Supervise Certified Nursing Assistants (CNAs) and participate in their evaluation.
• Oversee Falls Program and Pressure Sore Program.
• Provide nursing services.
• Conduct and document a thorough evaluation of each resident’s medical status upon admission and throughout the resident’s course of treatment.
• Other duties as assigned
