Job summary

HealthCare Partners of South Carolina is seeking an experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care under physician or nurse supervision in a clinic setting. You will assess patients, administer medications, perform procedures, and educate patients and families while coordinating care within an outpatient team.

The role requires SC LPN licensure and BLS certification, with 1+ year of outpatient or clinic-based nursing preferred.

Qualifications

  • Graduate of an accredited Practical Nursing program.
  • Current, unrestricted South Carolina LPN license.
  • Current BLS certification from American Heart Association or Red Cross.
  • Minimum of 1 year of clinical nursing experience preferred.
  • Experience with EHR systems and Microsoft Office preferred.

Responsibilities

  • Prepare patients for examinations by documenting histories, vitals, and complaints in the EHR.
  • Assess patient needs through observation and review of diagnostic information, reporting findings.
  • Provide direct patient care including assessments, treatments, medications, immunizations, wound care, and other nursing procedures.
  • Perform clinical procedures such as EKGs, specimen collection, point-of-care testing, and inhalation treatments.
  • Assist physicians with examinations, treatments, and minor procedures.
  • Educate patients and families on medications, preventive care, and follow-up plans.
  • Coordinate labs, referrals, and follow-up appointments as directed.
  • Conduct telephonetriage and follow-up calls as appropriate.

Position Summary

The Licensed Practical Nurse (LPN) provideshigh-quality nursing care under the direction of a physician, advanced practiceprovider, or Registered Nurse in accordance with the South Carolina NursePractice Act. The LPN is responsible for assessing patient needs, deliveringdirect patient care, assisting with examinations and procedures, administeringmedications and treatments, coordinating care, and educating patients and theirfamilies. This position plays a vital role in supporting the mission of HealthCare Partners of South Carolina by delivering compassionate, patient-centeredcare while promoting positive health outcomes for the communities we serve.

Essential Duties and Responsibilities
  • Prepare patientsfor examination by obtaining and documenting medical histories, chiefcomplaints, vital signs, and other clinical information in the ElectronicHealth Record (EHR).
  • Assess patientneeds through observation, communication, physical assessment, telephonetriage, and review of diagnostic information, reporting significant findings tothe provider or Registered Nurse.
  • Provide directpatient care, including assessments, treatments, medication administration,immunizations, injections, wound care, dressing changes, and other nursingprocedures within the LPN scope of practice.
  • Perform clinicalprocedures including specimen collection, electrocardiograms (EKGs),point-of-care testing, inhalation treatments, and other delegated duties asappropriate.
  • Assist physiciansand advanced practice providers with examinations, treatments, and minorprocedures.
  • Educate patientsand families regarding medications, preventive care, chronic diseasemanagement, treatment plans, discharge instructions, and follow-up care.
  • Coordinatelaboratory testing, referrals, diagnostic procedures, and follow-upappointments as directed by the provider.
  • Conduct telephonetriage and follow-up calls, providing appropriate guidance within the scope ofpractice and escalating concerns to the provider when necessary.
  • Accuratelydocument patient assessments, treatments, communications, and clinicalactivities in the Electronic Health Record (EHR) in a timely manner.
  • Support qualityimprovement initiatives by participating in chart audits, quality assuranceactivities, and organizational initiatives, including HRSA Uniform Data System(UDS) measures.
  • Maintainexamination rooms, medical equipment, and clinical supplies while ensuringcompliance with OSHA, infection prevention, and safety standards.
  • Monitor medicalsupplies, medications, and vaccines for inventory levels and expiration dates, and notify appropriate personnel of equipment or supply needs.
  • Maintain patientconfidentiality and protect health information in accordance with HIPAA andorganizational policies.
  • Respondappropriately to medical emergencies following established clinical protocols.
  • Collaborate withproviders, nursing staff, and interdisciplinary care teams to ensurecoordinated, patient-centered care.
  • Participate inrequired trainings, continuing education, staff meetings, and organizationalquality improvement initiatives.
  • Travel betweenHealth Care Partners of South Carolina clinic locations as needed.
  • Perform otherduties as assigned.
Education & Experience
  • Graduate of anaccredited Practical Nursing program required.
  • Current,unrestricted South Carolina Licensed Practical Nurse (LPN) license required.
  • Current BasicLife Support (BLS) certification through the American Heart Association orAmerican Red Cross required.
  • Minimum of one(1) year of clinical nursing experience in a physician practice, outpatientclinic, community health center, or similar healthcare setting preferred.
  • Experienceworking in a Federally Qualified Health Center (FQHC) or community healthcenter preferred.
  • Experience usingElectronic Health Record (EHR) systems and Microsoft Office preferred.
Knowledge, Skills, and Abilities
  • Knowledgeofpractical nursing principles, clinical procedures, medication administration,and infection prevention practices.
  • KnowledgeofHIPAA regulations and the ability to maintain the confidentiality, security,and integrity of patient information.
  • Strongassessment, organizational, and critical thinking skills with the ability toprioritize patient care in a fast-paced clinical environment.
  • Excellentcustomer service, communication, and interpersonal skills with a commitment toproviding compassionate, culturally competent, patient-centered care.
  • Proficiency inMicrosoft Office and Electronic Health Record (EHR) systems.
  • Ability toexercise sound clinical judgment, respond appropriately in emergencysituations, and establish effective working relationships with patients,providers, leadership, and coworkers.
