Affirmed Home Care, Inc. is seeking experienced Licensed Practical Nurses (LPNs) for flexible per diem home care in New Jersey, including Clifton and Passaic County. Earn $45.00/hour for nursing shifts and $85.00 per LPN visit, with options for weekdays, weekends, days, and overnights.

Requirements include NJ LPN license, 1 year of relevant nursing experience, BLS, US work authorization, and a medical exam within 12 months. Flu vaccination proof or exemption is required.

Qualificações

• Active New Jersey LPN license.
• Minimum of 1 year of nursing experience in acute care or home care.
• Current BLS certification.
• Authorization to work in the United States.
• Physical examination completed within the past 12 months.
• PPD or QuantiFERON completed within the past 12 months, or chest X-ray within the past 5 years.
• Proof of flu vaccination or an approved medical exemption.
• Valid driver’s license preferred.

Responsabilidades

• Provide one-on-one home nursing care to patients.
• Document and coordinate care with the clinical team.
• Assignments based on patient needs and nurse availability.
• Administer medications and monitor patient vitals per plan of care.

About the Opportunity

Scheduling Opportunities

We are looking for LPNs with availability for:

• Weekday shifts
• Weekend shifts
• Day shifts
• Overnight shifts

Assignments are based on patient needs and nurse availability.
