Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. As a LPN at Van Duyn, you will play a vital role in providing high-quality patient care, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities:

• Administer medications as prescribed by physicians
• Serve as charge nurse, overseeing the care provided by Certified Nursing Assistants (CNAs)
• Monitor patient/resident plans of care and participate in the care planning process
• Report to the nurse manager, nursing supervisor, or Assistant Director of Nursing (ADON)
• Collaborate with the healthcare team to deliver excellent patient care

Requirements/Qualifications:

• Valid LPN Practical Nurse License
• CPR Certification
