Job summary

The Enclave at Rye Rehabilitation and Nursing Center is seeking a Licensed Practical Nurse (LPN) to care for residents in our Skilled Unit and Long Term Care Unit. You will administer medications (including IVs) and monitor outcomes, take vital signs, and provide comprehensive resident care with education for residents and families.

Communicate with physicians and care teams, and supervise CNAs to ensure high-quality care in a supportive environment with recognition programs and uniforms

Qualifications

• Active LPN license in good standing.
• Experience in SNF/long-term care preferred.
• Ability to work in a fast-paced environment.

Responsibilities

• Medication administration, including IVs.
• Take and record vital signs.
• Provide resident care and treatments with outcome monitoring.
• Educate residents and families.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs.
