Job summary

The Enclave at Rye Rehabilitation and Nursing Center in Lattingtown, NY is seeking a Licensed Practical Nurse (LPN) to join our skilled unit and long-term care team. You will administer medications, including IVs, take vital signs, and provide direct resident care with ongoing monitoring of outcomes.

You will educate residents and families, communicate with physicians and the care team, and supervise CNAs to ensure high-quality care is delivered daily.

Qualifications

• Current LPN license in good standing.
• Experience in skilled nursing or long-term care is preferred.

Responsibilities

• Provide care for residents in SNF and long-term care units.
• Administer medications, including IVs, and monitor outcomes.
• Take and record vital signs and document changes.
• Deliver resident and family education and support.
• Coordinate with physicians, NPs, families, and healthcare team.
• Supervise CNAs and ensure quality of care.
