Job summary

Chatham Hills Subacute is seeking a Licensed Practical Nurse (LPN) for day shift in our skilled nursing and long-term care units. You will provide direct patient care and manage routine nursing tasks with a focus on resident comfort and safety.

Responsibilities include medication administration (including IVs where applicable), taking vital signs, supervising CNAs, and communicating with physicians, NPs, families, and care teams. An LPN license is required.
