Job summary

Care Options for Kids is seeking a Licensed Practical Nurse (LPN) to provide dedicated, one-on-one pediatric home health care in New Jersey. In this role you will focus on delivering consistent, high-quality care with support from a registered nurse and clinical leadership.

You will work closely with families, follow individualized care plans, and maintain safety standards including PPE and CDC guidelines. NJ LPN license and active CPR are required.

Qualifications

• Valid New Jersey LPN License or Multistate License.
• Current physical from within three years.
• PPD or Chest X-Ray.
• BLS CPR card (obtained in person).
• Valid driver’s license.
• G-tube, trach, vent experience or willingness to train.

Responsibilities

• Provide one-on-one pediatric home health care.
• Collaborate with RN and clinical leadership to follow plans of care.
• Deliver skilled nursing care with safety and CDC guidelines in every visit.
• Support families and ensure continuity of care in the home setting.
