Job summary

Bradford Manor Nursing and Rehabilitation is seeking a compassionate Licensed Practical Nurse (LPN) to join our evening/overnight shift team. You will provide high-quality skilled nursing and rehabilitation services to our residents in a supportive, patient‑focused environment.

Responsibilities include administering medications, wound and IV care as appropriate, monitoring vital signs, and collaborating with the interdisciplinary team to implement care plans.

Qualifications

• Current PA LPN licensure is required.
• HS diploma or equivalent.
• 1+ year of experience in long-term care or rehabilitation (preferred).

Responsibilities:

• Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Monitor and report patient conditions, including vital signs, to the charge nurse and physicians
• Assist with tube feedings, IV therapy, and other treatments as needed
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
• Participate in quality improvement initiatives to continually improve patient outcomes and satisfaction

Requirements:

• Current LPN licensure in the state of Pennsylvania
• High school diploma or equivalent required
• 1+ years of experience in long-term care or rehabilitation setting (n/a)
