Birmingham Rehabilitation and Wellness Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in Forestdale, AL. The role operates Monday-Friday with two shift options and Baylor weekend coverage, contributing to a fast-paced rehabilitation setting.

The LPN will support interdisciplinary teams, provide patient care under supervision, administer medications, and assist with education and discharge planning while maintaining accurate records and a safe environment.

Qualifications

• Current LPN licensure in the State of Alabama.
• CPR Certification required.

Responsibilities

• Provide direct patient care under RN supervision, including medications and wound care.
• Develop and implement individualized care plans with the interdisciplinary team.
• Maintain accurate patient records and documentation.
• Participate in patient education and discharge planning.
• Collaborate with physical, occupational, and speech therapists for rehabilitation.
• Maintain a safe and clean environment for patients and staff.

Shifts Available: Monday-Friday (NO WEEKENDS)

2:30pm-11:00pm; 6:30am-3:00pm

Baylor Weekends: 6:30pm-7:00am (Saturday & Sunday)

Job Summary:

Birmingham Rehabilitation and Wellness Center is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As an integral part of our interdisciplinary team, the LPN will provide high-quality patient care and support in a dynamic and fast-paced rehabilitation setting.

Responsibilities:

• Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and performing vital signs

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Maintain accurate and up-to-date patient records and documentation

• Participate in patient education and discharge planning to ensure a smooth transition to home or community-based care

• Collaborate with physical, occupational, and speech therapists to ensure comprehensive patient rehabilitation

• Maintain a safe and clean environment for patients, staff, and visitors

Requirements:

• Current LPN licensure in the State of Alabama

• CPR Certification
