Job summary

Birmingham Rehabilitation and Wellness Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our rehabilitation team in Forestdale, AL.

You'll provide direct patient care under RN supervision, administer medications, assist with wound care, monitor vital signs, and collaborate with therapists to support patient recovery in a fast-paced setting. Shifts include 2:30pm-11:00pm and 6:30am-3:00pm, with Baylor Weekends options.

Qualifications

• Current LPN licensure in Alabama.
• CPR Certification is required.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and performing vital signs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Maintain accurate and up-to-date patient records and documentation
• Participate in patient education and discharge planning to ensure a smooth transition to home or community-based care
• Collaborate with physical, occupational, and speech therapists to ensure comprehensive patient rehabilitation
• Maintain a safe and clean environment for patients, staff, and visitors
