Job summary

St. Joseph Hospice in Lake Charles, LA is hiring a Full-Time PRN Hospice Registered Nurse (RN) to provide compassionate, patient-centered care in a home or facility setting.

You will conduct assessments, develop and implement the Plan of Care, educate patients and families, coordinate with physicians and the hospice team, and maintain accurate records. Qualifications

• Licensed RN in Louisiana (required).
• Minimum 2 years full-time RN experience in oncology, acute care with direct patient care, long-term care, hospice or home health. Hospice experience preferred.
• CPR certification is required.
• Current automobile insurance and valid driver's license.
• Knowledge of EMR (electronic medical records) is required.

Responsibilities

• Perform initial and ongoing patient assessments.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care, medications, dressings, and Hospice philosophy.
• Act as primary liaison between physician, patient/family and hospice team.
• Maintain accurate and updated medical records.
• Make routine supervisory visits and participate in on-call schedule.
• Adhere to company policies, compliance guidelines, and report any compliance concerns promptly.

Qualifications:

• Registered Nurse (RN) license required in State of Louisiana
• Minimum of two (2) years full-time experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care. Two (2) of experience in hospice care as an LPN may be substituted for the required 2 years of experience as an RN
• Current CPR certification required
• Current automobile insurance and valid driver’s license
• Knowledge of EMR is required
• Previous Hospice experience preferred
• Compassionate RN with great time management skills

Job Duties:

• Performs the initial and on-going assessment of the patient

• Act as primary liaison between physician, patient/family and hospice team
