Northgate Health and Rehabilitation Center in Bessemer, AL is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing team. You will provide direct resident care, administer medications, and support our clinical staff in delivering high-quality care.

The role requires an Alabama LPN license, experience in long-term care or rehab, and strong clinical assessment, communication, and teamwork skills.

Qualifications

• Active LPN license in Alabama.
• Experience in long-term care, skilled nursing, or rehab preferred.
• Strong clinical and assessment skills.
• Excellent communication and teamwork abilities.
• Ability to prioritize and work in a fast-paced environment.
• Willingness to work weekends and holidays as needed.

Responsibilities

• Provide direct nursing care according to individualized care plans.
• Administer medications and treatments as ordered.
• Monitor resident condition including vital signs and changes in status.
• Complete accurate and timely documentation.
• Assist with admissions, transfers, and discharges.
• Work closely with RNs, CNAs, physicians, and therapy staff.
• Support residents with daily care needs when needed.
• Participate in care planning and family meetings.
• Follow infection control and safety procedures; help train and support new nursing staff.

LPN Requirements

• Active Licensed Practical Nurse (LPN) license in Alabama
