Job summary
Northgate Health and Rehabilitation Center in Bessemer, AL is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing team. You will provide direct resident care, administer medications, and support our clinical staff in delivering high-quality care.
The role requires an Alabama LPN license, experience in long-term care or rehab, and strong clinical assessment, communication, and teamwork skills.
Qualifications
- Active LPN license in Alabama.
- Experience in long-term care, skilled nursing, or rehab preferred.
- Strong clinical and assessment skills.
- Excellent communication and teamwork abilities.
- Ability to prioritize and work in a fast-paced environment.
- Willingness to work weekends and holidays as needed.
Responsibilities
- Provide direct nursing care according to individualized care plans.
- Administer medications and treatments as ordered.
- Monitor resident condition including vital signs and changes in status.
- Complete accurate and timely documentation.
- Assist with admissions, transfers, and discharges.
- Work closely with RNs, CNAs, physicians, and therapy staff.
- Support residents with daily care needs when needed.
- Participate in care planning and family meetings.
- Follow infection control and safety procedures; help train and support new nursing staff.
