Job summary

Summa Home Health is hiring a Licensed Practical Nurse to provide one-on-one skilled nursing care in patients' homes in the Akron area. You will follow care plans, monitor conditions, and administer medications while educating families to support recovery.

Qualifications

• Current Licensed Practical Nurse (LPN) license.
• Minimum of 1 year of LPN experience in an acute care setting.
• Home health experience preferred.

Responsibilities

• Provide skilled nursing care in patients' homes.
• Follow physician orders and care plans created by the Case Manager.
• Monitor patient conditions and report changes.
• Administer medications and treatments as prescribed.
• Educate patients and families using teach-back methods.
• Help reduce hospital readmissions through patient education and preventative care.
• Complete clinical documentation in accordance with agency and Medicare guidelines.
• Communicate schedule or availability changes with the appropriate team members.

What You'll Do
