Job summary

Sunrise Senior Living in Virginia is seeking a dedicated Licensed Practical Nurse (LPN) to support resident health and wellbeing across our community. In this role, you will monitor health, verify medication administration, document changes, and collaborate with families and physicians to maintain high clinical quality.

Ideal candidates hold an active LPN license in good standing, have 1+ year in senior care or clinical settings, and excel in assessment, documentation, and communication.

Qualifications

• Graduate of approved nursing program.
• 1+ year experience in senior care or clinical setting.
• Strong assessment, documentation, and communication skills.

Responsibilities

• Conduct move-in, 3-day, semi-annual, and change-of-condition assessments.
• Perform monthly wellness visits and monitor health trends.
• Follow up on falls, incidents, and hospital returns.
• Transcribe physician orders and ensure accurate MAR/TAR documentation.
• Complete medication audits and oversee medication compliance.
• Partner with families, physicians, and the Resident Care Director.
• Support infection control and regulatory standards.
• Educate and guide care staff on documentation and medication practices.
