Job summary

Sunrise Senior Living is seeking a Licensed Practical Nurse (LPN) to support resident health and wellbeing across the community. The role focuses on monitoring residents, medication oversight, and maintaining clinical quality with proactive assessments and timely follow-ups.

You will conduct move-in assessments, monthly wellness visits, document accurately, and collaborate with families, physicians, and the Resident Care Director to ensure high-quality care and regulatory compliance.

Qualifications

• Active LPN license in good standing.
• Graduate of approved nursing program.
• 1+ year experience in senior care or clinical setting.
• Strong assessment, documentation, and communication skills.

Responsibilities

• Conduct move-in, 3-day, semi-annual, and change-of-condition assessments
• Perform monthly wellness visits and monitor health trends
• Follow up on falls, incidents, and hospital returns
• Transcribe physician orders and ensure accurate MAR/TAR documentation
• Complete medication audits and oversee medication compliance
• Partner with families, physicians, and the Resident Care Director
• Support infection control and regulatory standards
• Educate and guide care staff on documentation and medication practices

Pay Range: 32.75 - 43.75
