A dynamic healthcare company seeks a compassionate Licensed Practical Nurse (LPN) to enhance Chronic Care Management and Remote Patient Monitoring programs. You will play a crucial role in patient assessments, care planning, and education to empower patients in managing their chronic conditions effectively while ensuring personalized care. This position emphasizes collaboration with a multidisciplinary team and aims to deliver high-quality healthcare services.
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to support our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. This role focuses on empowering patients to manage their chronic diseases effectively while ensuring high-quality care delivery. The ideal candidate will conduct comprehensive assessments, assist in developing care plans, and provide patient-centered education and support.
Key Responsibilities :
Perform thorough assessments of patients' medical, psychological, and social needs.
Assist in developing and implementing personalized care plans in collaboration with a multidisciplinary team, including physicians and pharmacists.
Educate patients on managing chronic diseases, including medication adherence, vital sign monitoring, nutrition, exercise, and lifestyle modifications.
Facilitate communication between patients, caregivers, and healthcare team members to ensure seamless coordination of care.
Monitor RPM data for trends or potential health risks and escalate findings to the appropriate care team members.
Accurately document patient interactions, care activities, and assessments in the electronic health record (EHR) system.
Assist in enrolling eligible patients into CCM and RPM programs.
Qualifications and Requirements :
Active Licensed Practical Nurse (LPN) license in good standing.
At least 2 years of experience in patient care, care coordination, or chronic disease management.
Proficiency in electronic health records (EHR) systems.
Strong interpersonal and communication skills for patient education and team collaboration.
Reliable internet connection and a HIPAA-compliant workspace for telephonic and virtual interactions.
