Job summary

Home Health Care, Inc. is seeking a dedicated LPN to perform 1:1 home visits across a 35-mile radius of your home in the Twin Cities area. You will travel between patients, delivering scheduled skilled nursing care and documenting your findings thoroughly.

You will assess conditions, administer wound care, educate patients and families, and collaborate with RN Case Managers to adjust plans of care as needed. This role emphasizes attentive, patient-centered nursing in the home setting.

You'll travel to patients throughout your local area and provide scheduled skilled nursing visits one patient at a time.

Work Close to Where You Live

Patient visits are scheduled within approximately a 35-mile radius of your home, helping create a service area centered around where you live.

You’ll spend your day traveling between patients in your local area while receiving mileage reimbursement between patient visits.

• Reinforcing physician orders and plans of care

• Reliable vehicle, valid driver's license, and current auto insurance
• Ability to travel to patients within approximately a 35-mile radius of your home
• Home health experience preferred, but not required
