Job summary

Home Health Care, Inc. is seeking a Licensed Practical Nurse (LPN) Visit Nurse to provide skilled nursing visits in patients' homes within a ~35-mile radius of your residence in the Twin Cities area.

This full-time role emphasizes one-on-one patient care, relationship building, and maintaining high clinical standards. You will collaborate with a clinical team, perform wound care and medication management, educate families, and document visits to ensure seamless continuity of care in a supportive

Qualifications

• Active Minnesota LPN license is required.
• Ability to work independently while providing compassionate, patient-centered care.
• Reliable vehicle, valid driver's license, and current auto insurance.

Responsibilities

• Provide skilled nursing visits in patients' homes within approximately a 35-mile radius of your home.
• Perform wound care, medication management, and other skilled nursing interventions.
• Educate patients and families on treatments, medications, and disease management.
• Collaborate closely with RN Case Managers, physicians, therapists, and the interdisciplinary care team.
• Document visits accurately and ensure continuity of care.

What We're Looking For

• Active Minnesota Licensed Practical Nurse (LPN) license.
  
• Ability to work independently while providing compassionate, patient-centered care.
  
• Reliable vehicle, valid driver's license, and current auto insurance.
  
• Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred.
  
• EMR experience is a plus, but we'll help you learn our systems.
