Home Health Care, Inc. seeks an LPN Visit Nurse to bring skilled nursing care into patients' homes within roughly a 35-mile radius of your Minnesota residence. You drive to patients, assess, treat, educate, and document care, with RN Case Managers and clinicians supporting you.
In this full-time role, you will provide 1:1 patient care, coordinate with interdisciplinary teams, and focus on helping patients remain safely at home. You’ll enjoy autonomy with clinical mentorship.

Your patient visits will be scheduled within approximately a 35-mile radius of your home, allowing you to serve patients in communities closer to where you already live.
