Job description

Job Title: Licensed Practical Nurse (LPN) | Home Health Visits | One Patient at a Time

Company: Home Health Care, Inc.

Location: Twin Cities Metro Area | Patient Visits Within Approximately a 35-Mile Radius of Your Home

Schedule: Full-Time

Compensation: $30–$38 per visit

Benefits: Comprehensive Benefits Package + Mileage & Cell Phone Reimbursement

Remember When Nursing Was About the Patient in Front of You? Not the call light going off down the hall. Not the medication pass you're already behind on. Not wondering how you're going to get everything done before the end of your shift. Just you and the patient who needs you. As an LPN Visit Nurse with Home Health Care, Inc., that's what your workday is built around. You'll travel to patients throughout your local area and provide scheduled skilled nursing visits one patient at a time. Assess what you're seeing. Provide the treatment. Answer the questions. Educate the family. Make sure they understand what happens next. Then move on to your next patient. Sometimes better nursing starts with simply having the opportunity to focus. You're Not Just There to Complete a Task A dressing change might be why the visit was scheduled. But when you walk through the door, you may notice something else. The patient doesn't understand their new medication. A caregiver has questions about the treatment plan. The wound doesn't look like it did last week. The patient mentions a symptom they hadn't reported. Something about their condition simply doesn't look right. Home health gives you the opportunity to observe, educate, communicate, and advocate while providing the skilled nursing care the patient needs. Because great nursing isn't just about completing the ordered treatment. It's about paying attention to the person receiving it.

Work Close to Where You Live

Your territory shouldn't automatically mean the entire Twin Cities. Patient visits are scheduled within approximately a 35-mile radius of your home, helping create a service area centered around where you live. You'll spend your day traveling between patients in your local area while receiving mileage reimbursement between patient visits.

What Your Visits Could Include
  • Skilled nursing observations and assessments
  • Wound care and dressing changes
  • Medication management and education
  • Injections and ordered treatments
  • Chronic disease monitoring
  • Patient and caregiver education
  • Reinforcing physician orders and plans of care
  • Monitoring for changes in patient condition
  • Communicating concerns to RN Case Managers and physicians
  • Collaborating with therapists and other interdisciplinary clinicians
  • Accurate and timely clinical documentation

Every visit is different, but the goal remains the same: Help the patient remain healthy, informed, independent, and safely at home.

Why LPNs Choose This Role
  • Provide 1:1 patient care
  • Visits within approximately a 35-mile radius of your home
  • More opportunity to focus on the patient in front of you
  • Build ongoing relationships with patients and caregivers
  • Practice independently in the field
  • Use your nursing judgment and observation skills
  • Work alongside experienced RN Case Managers and Clinical Managers
  • Help patients successfully recover and manage their health at home
Benefits That Support You in the Field
  • $30–$38 per visit
  • Employer contributions toward medical insurance
  • Employer-paid dental, vision, life insurance, and disability coverage
  • Mileage reimbursement between patient visits
  • Cell phone reimbursement for eligible clinicians
  • Free access to our MedBridge Education Hub
  • Licensure reimbursement at renewal
  • Annual scrub reimbursement
  • Three weeks of PTO plus six paid holidays
  • Structured orientation and ongoing clinical support
  • Career opportunities throughout our family of healthcare companies
Bring the LPN Experience You Already Have

Home health experience is valuable, but it isn't the only background that can prepare you for this role. If you've worked in:

  • Skilled nursing TCU or rehabilitation
  • Assisted living
  • Hospice Clinic nursing
  • Other direct patient-care environments you've already developed skills that can translate well into home health. We'll help you learn the home health environment while you bring the clinical experience, communication skills, and patient instincts you've already built.
What We're Looking For
  • Active Minnesota Licensed Practical Nurse (LPN) license
  • Strong clinical observation and patient-care skills
  • Excellent communication and organization
  • Ability to work independently in patients' homes
  • Compassionate and dependable approach to care
  • Reliable vehicle, valid driver's license, and current auto insurance
  • Ability to travel to patients within approximately a 35-mile radius of your home
  • Home health experience preferred, but not required
Why Home Health Care, Inc.?

Since 1994, Home Health Care, Inc. has helped Minnesota patients receive skilled healthcare in the place they know best—their own homes. We're locally owned, Medicare-certified, and backed by more than 30 years of home health experience. Our field clinicians have the independence to manage their visits while still having experienced clinical leadership and an interdisciplinary team available when they need support.

Give Your Next Patient Your Attention—Not What's Happening in Six Other Rooms. One patient. One conversation. One set of clinical needs. One opportunity to notice something that could make a difference. Then it's on to the next home. If that's the kind of nursing day you've been looking for, we'd like to meet you.

Full-Time. $30–$38 per visit. Patient visits within approximately a 35-mile radius of your home.

One patient at a time. That's where great care starts.
