RHA Health Services, LLC in Chattanooga, TN is hiring a Licensed Practical Nurse (IDD LPN) for an on-site, office-based role supporting group home operations. Monday–Friday, 8:30am–5:00pm with travel between group homes as needed for resident assessments.

The position emphasizes structured, steady-paced nursing outside hospital settings, with prior IDD and case management experience encouraged.

Responsibilities

• Provide holistic nursing care to individuals with IDD.
• Observe and report health needs to the RN.
• Coordinate preventive and routine medical care.
• Document nursing care provided.
• Assist physicians with on-site clinics.
• Provide health trainings to unlicensed staff.
• Conduct medication pass observations for staff competency.
• Perform medication closest assessments.
• Run QA checks to ensure staff perform health tasks correctly.
• Report abnormal findings to physicians.
• Communicate with physicians, pharmacies, suppliers, and the IDT.
• Advocate for people supported and develop community supports.

We are hiring for: Licensed Practical Nurse (IDD LPN) - Office Days Type: Regular

Location: Chattanooga, TN (On-Site - must willing to travel between group homes within Chattanooga for resident asessment)

Employment Type: Full-time Incentives: No sign-on bonus offered. No relocation packaged offered. Schedule: Monday - Friday, 8:30am - 5:00pm This is an office-based position that supports our group home operations and requires routine visits to our residential homes. It is ideal for nurses seeking a more structured and steady-paced environment outside of traditional fast-paced hospital or SNF settings. Candidates with prior IDD and case management experience are strongly encouraged to apply.

REPORTS TO: Regional Nursing Director

SUPERVIORY RESPONSIBILITIES: This position has no supervisory responsibilities.

Job Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities
• Observe, assist with gathering health related data, and report the health needs or the people we support to the registered nurse
• Coordinate preventative and routine medical care for people supported
• Document nursing care provided
• Assist physicians with on-site medical clinics where applicable
• Provide health related trainings to unlicensed personnel according to state regulations
• Conduct medication pass observations in the home to ensure staff competency according to state regulations
• Conduct medication closest assessments
• Perform quality assurance checks and observations to ensure unlicensed personnel are performing health related tasks as assigned according to state regulations
• Observe for and report any abnormal health related findings to the physician
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
• Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported

MINIMUM QUALIFICATIONS

• Graduation from an accredited licensed nurse program.
• Current LPN license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to certify for CPR.
• Possess credentials required by Federal or State law
• Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families.
• Must be able to work independently, be organized, and possess the ability to prioritize work tasks.
• Must be at least 18 years of age

PHYSICAL DEMANDS AND WORK ENVIRONMENT

• Must be able to regularly lift and carry up to 20 lbs. and occasionally pull at least 40 lbs.
• Must be able to squat, kneel, crawl, crouch, climb, and stoop as part of regular job duties.
• Must be able to use hands and fingers to handle or operate objects, tools, or controls.
• Required to stand and walk for extended periods.
• Must be able to demonstrate proficiency in CPR from floor level, which requires working on hands and knees, bending, standing, and lifting.
• Vision requirements include close, distance, and peripheral vision.
• Must be able to talk and hear.
• Exception – Deaf and Hard of Hearing Programs: For these programs only, employees must be fluent in American Sign Language (ASL). The requirement to talk and hear does not apply.
