Job summary

The LVN will document visits in the EMR, participate in coordination activities, and adapt to diverse client needs while maintaining professional standards.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to dependable transportation or public transit.
• CPR certification or ability to obtain within 90 days of hire.
• Ability to manage multiple tasks, work independently and communicate effectively.

Responsibilities

• Provides high quality clinical services within the scope of practice and plan of care.
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements.
• Participates in care coordination, reporting changes in condition and communicating with patient, family, physicians and team.
• Demonstrates adaptability to varied home environments and client needs.
