Job summary

Hartford HealthCare at Home in Torrington, CT seeks a Revisit Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients' homes and community settings under the direction of an RN. The role requires covering Torrington and surrounding towns with one weekend per month.

This role will collaborate with all team members to achieve exceptional outcomes.Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.Records daily progress in EMR.Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.Communicates/conferences with primary nurse and other disciplines.Consults with physician regarding client needs and plan of care.Assists in evaluation of care given by paraprofessional staff.Participates on agency committees and special projects.

Qualifications

Minimum Requirements:- High School diploma or GED- Successful completion of a CT state approved LPN certificate programExperience- Computer skills including but not limited to Outlook, Microsoft Word, and ExcelLicensure- Current and valid LPN license- Valid CT driver’s license & a good driving history
