Job summary

Hartford HealthCare at Home is seeking a Revisit Licensed Practical Nurse (LPN) to provide skilled nursing care in clients’ homes and community settings under RN supervision. This role covers Torrington and surrounding towns, with occasional weekends and major/minor holidays, offering a dynamic, patient-centered care model.

• Provide intermittent skilled nursing services under the direction of an RN.
• Employ teaching principles to promote health and prevent disease.
• Record daily progress in EMR.
• Report to primary nurse, supervisor, physician, social worker, or therapist.
• Communicate with primary nurse and other disciplines.
• Consult with physician regarding client needs and plan of care.
• Assist in evaluating care provided by paraprofessional staff.
• Participate in agency committees and special projects.

Shift Detail: 8-4:30 Mon-Fri. Cover Torrington and surrounding towns. Cover 1 weekend a month, 1 major and 2 minor holidays a year. Work whereevery momentmatters.

• Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.
• Communicates/conferences with primary nurse and other disciplines.
• Consults with physician regarding client needs and plan of care.
• Assists in evaluation of care given by paraprofessional staff.
• Participates on agency committees and special projects.

Qualifications

• High School diploma or GED
• Successful completion of a CT state approved LPN certificate programExperience
• Computer skills including but not limited to Outlook, Microsoft Word, and ExcelLicensure
• Current and valid LPN license
• Valid CT driver’s license & a good driving history
