Job summary

Hartford HealthCare at Home is seeking a Revisit Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes and community settings, under the direction of an RN.

The role requires CT LPN licensure, a CT driver’s license with a good driving history, and the ability to cover Torrington and surrounding towns, including weekends and holidays.

Qualifications

• High School diploma or GED.
• CT state approved LPN certificate program.
• Experience with Outlook, Word and Excel.
• Current and valid CT LPN license.
• CT driver’s license & a good driving history.

Responsibilities

• Provide skilled nursing care to clients in home-based settings under the direction of an RN.
• Collaborate with care team to achieve outcomes.
• Record daily progress in EMR and report to nursing supervisor and physician.
• Confer with primary nurse and other disciplines as needed.
• Assist in evaluation of care delivered by paraprofessional staff.

This role will collaborate with all team members to achieve exceptional outcomes.Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.Records daily progress in EMR.Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.Communicates/conferences with primary nurse and other disciplines.Consults with physician regarding client needs and plan of care.Assists in evaluation of care given by paraprofessional staff.Participates on agency committees and special projects.

Qualifications

Minimum Requirements:- High School diploma or GED- Successful completion of a CT state approved LPN certificate programExperience- Computer skills including but not limited to Outlook, Microsoft Word, and ExcelLicensure- Current and valid LPN license- Valid CT driver’s license & a good driving history
