Job summary

Hartford HealthCare at Home seeks a Revisit Licensed Practical Nurse (LPN) to deliver skilled nursing services in clients’ homes across Torrington and surrounding towns. Under RN supervision, you will implement the established care plan and promote health and independence.

Ideal candidates hold a CT LPN certificate, a valid CT driver's license, and can document progress in EMR. This full-time position emphasizes teamwork, community-based care, and quality outcomes.

Qualifications

• High School diploma or GED.
• Successful completion of a CT state approved LPN certificate program.
• CT driver’s license with good driving history.

Responsibilities

• Provide intermittent skilled nursing services under the direction of an RN.
• Record daily progress in EMR and communicate with the primary nurse.
• Collaborate with physicians, social workers and other disciplines as needed.
• Assist in evaluation of care and participate in agency committees.

This role will collaborate with all team members to achieve exceptional outcomes.Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.Records daily progress in EMR.Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.Communicates/conferences with primary nurse and other disciplines.Consults with physician regarding client needs and plan of care.Assists in evaluation of care given by paraprofessional staff.Participates on agency committees and special projects.

Qualifications

Minimum Requirements:- High School diploma or GED- Successful completion of a CT state approved LPN certificate programExperience- Computer skills including but not limited to Outlook, Microsoft Word, and ExcelLicensure- Current and valid LPN license- Valid CT driver’s license & a good driving history
