Job summary

Hartford HealthCare at Home seeks a Revisit Licensed Practical Nurse (LPN) to deliver skilled nursing care in clients’ homes and communities. Under RN supervision, you’ll follow the established plan of care to promote health and prevent disease, coordinating with the care team and documenting progress in the EMR.

The role requires CT LPN licensure, a CT driver’s license with a good driving record, and computer skills to manage patient records and communications.

Qualifications

• High School diploma or GED.
• CT state approved LPN certificate program.
• Current and valid LPN license.

Responsibilities

• Provide intermittent skilled nursing services in the home under the direction of an RN.
• Record daily progress in the EMR system.
• Communicate with the primary nurse, supervisor, physician, social worker or therapist regarding cases.

This role will collaborate with all team members to achieve exceptional outcomes.Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.Records daily progress in EMR.Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.Communicates/conferences with primary nurse and other disciplines.Consults with physician regarding client needs and plan of care.Assists in evaluation of care given by paraprofessional staff.Participates on agency committees and special projects.

Qualifications

Minimum Requirements:- High School diploma or GED- Successful completion of a CT state approved LPN certificate programExperience- Computer skills including but not limited to Outlook, Microsoft Word, and ExcelLicensure- Current and valid LPN license- Valid CT driver’s license & a good driving history
