Qualifications

As a Home Health LPN, You Will:

• Provide professional nursing services under the supervision of a RN, in compliance with all relevant laws, standards, and policies.
• Deliver skilled nursing interventions, including treatment, rehabilitative needs, and preventative care, using a holistic approach.
• Apply clinical knowledge and skills to facilitate problem resolution and meet patient goals.
• Coordinate with the RN Supervisor on the Plan of Treatment and communicate with physicians as permitted by law.
• Implement the nursing plan of care with a goal/outcome-driven approach.
• Maintain contact with patients, families, and healthcare team members regarding needs and changes.
• Ensure confidentiality and professional conduct in all information sharing.
• Communicate effectively to ensure proper supplies and safety measures are in place.
• Document visits and care activities accurately and timely.

Required Experience/Skills:

• Graduate of an accredited LPN/VN program.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of clinical experience as an LPN/LVN, preferably in home health or hospice.

Pay Range: $28.00 - $40.00 per visit/unit, or $44,600 - $61,400 annually for full-time.

Scheduled Weekly Hours: 40
