Pay Range

This range is provided by CenterWell Home Health. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.

Base pay range: $44,600.00/yr - $61,400.00/yr

As a Home Health LPN, You Will:

• Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirement, healthcare standards, laws, and policies.
• Provide skilled nursing interventions for treatment, rehabilitative needs, and preventative care, utilizing a holistic approach.
• Apply clinical knowledge and skills to facilitate problem resolution and achieve patient goals.
• Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate orders from physicians as permitted by law.
• Implement the nursing plan of care with a goal/outcome driven approach.
• Maintain contact with patients, physicians, and healthcare team regarding patient needs and status changes.
• Maintain confidentiality and share information only when appropriate.
• Communicate effectively to ensure proper supplies and equipment are available.
• Document visits and notes accurately and timely.

Required Experience/Skills:

• Graduate of an accredited LPN program or vocational nursing school.
• Current nursing license in practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of clinical experience as an LPN/LVN, preferably in home health or hospice.

Additional Details:

Pay per visit/unit: $28.00 - $40.00

Full-time annual base pay: $44,600 - $61,400
