As a Home Health LPN, you will:

1. Provide professional nursing services under the supervision of an RN, in compliance with the state's Nurse Practice Act, licensure requirements, healthcare standards, laws, regulations, and company policies.
2. Administer skilled nursing interventions for treatment, rehabilitation, and prevention, using a holistic approach aligned with the care plan.
3. Apply clinical knowledge and skills to facilitate problem resolution and achieve patient-specific outcomes.
4. Discuss necessary changes in the Plan of Treatment with the RN Supervisor and communicate verbal orders from physicians, as permitted by law.
5. Implement the nursing plan of care using a goal/outcome-driven approach.
6. Maintain communication with patients, physicians, and team members regarding patient needs and status, and participate in care coordination and discharge planning.
7. Uphold confidentiality and share information only when appropriate and in the best interest of the patient.
8. Ensure the availability of supplies and equipment needed for safe and effective care.
9. Document visits and care activities accurately and timely as per company and facility standards.

Use your skills to make an impact

Required Experience/Skills:

1. Graduate of an accredited LPN/Vocational Nursing program.
2. Current nursing license in the practicing state.
3. Valid driver’s license, auto insurance, and reliable transportation.
4. Current CPR certification.
5. At least two years of clinical experience as an LPN/LVN, preferably in home health or hospice settings.

Pay Range and Benefits

Starting pay per visit/unit ranges from $28.00 to $40.00, with an annual base pay of $44,600 to $61,400, depending on location and experience. Benefits include paid time off, 401(k), health insurance, wellness rewards, and recognition programs, among others.
