Pay Range

Base pay range: $27.00/hr - $31.00/hr

Job Overview

Medi Home Health Agency, a division of Medical Services of America, Inc., is seeking a Part-Time/PRN Home Health Licensed Practical Nurse (LPN) for our Home Health patients in Pulaski (Carroll, Floyd, Christiansburg, Blacksburg, Wythville), VA.

As a member of the multidisciplinary team, the LPN performs selected nursing care under the supervision of a registered nurse.

Responsibilities

• Provides skilled nursing care to clients as directed by the registered nurse and physician’s plan of care, in accordance with agency policies.
• Adheres to policies and procedures.
• Assists in planning and exchanging information with other health professionals to ensure continuity of care.
• Prepares clinical progress notes and follows client care plans developed by the registered nurse and agency policies.
• Participates in on-call rotation.

Job Requirements

• Successful completion of a certified licensed practical nursing program.
• Current VA LPN license.
• Minimum of one year of nursing experience.
• Home Health experience preferred.
• Reliable transportation and auto liability insurance.
