Job summary

Community Home Health of Maryland is seeking a dedicated Full-Time Licensed Practical Nurse (LPN) to support home health patients in Glen Burnie, Annapolis, Lanham, and Hyattsville (Anne Arundel, Prince Georges) MD. You will work within a multidisciplinary team to deliver skilled nursing care per plan of care.

The ideal candidate holds an MD LPN license, has at least 1 year of nursing experience, and is adaptable for on-call rotations. Home health experience is strongly preferred.

Qualifications

• Certified LPN program completed.
• Current MD LPN license.
• Minimum one year nursing experience.
• Home Health experience preferred.

Responsibilities

Community Home Health of Maryland, a division of Medical Services of America Inc., is currently seeking a passionate, self-motivated, experienced Full-Time Licensed Practical Nurse (LPN) for our Home Health Patients in Glen Burnie, Annapolis, Lanham, and Hyattsville (Anne Arundel, Prince Georges), MD.

Responsibilities:

As a member of the multidisciplinary team, the LPN is responsible for performing selected nursing care under the direction of a registered nurse.

• Provides skilled nursing care to clients as directed by the registered nurse and physician’s plan of care in accordance with agency policies.
• Adherence to policies and procedure.
• Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals.
• Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy.
• On-call rotation.

Job Requirements

• Successful completion of a certified licensed practical nursing program.
• Currently licensed as an LPN in the state of MD.
• Minimum of one year of nursing experience.
• Home Health experience strongly preferred.
• Reliable transportation and company required auto liability insurance.
