BAYADA Home Health Care

Licensed Practical Nurse (LPN) is sought to join our team in the Concord office. This office services adult clients on a per visit basis in homes throughout Keene and surrounding towns, NH. One year of prior clinical experience as a Licensed Practical Nurse is required.

As a home care nurse, you will be an integral part of a multidisciplinary healthcare team providing skilled nursing and rehabilitative care, enabling clients to receive necessary medical care at home.

LPN Responsibilities:

• Making home visits to clients in designated areas.
• Administering medication, wound care, treatments, and procedures.
• Monitoring client conditions and reporting changes.
• Following doctors' orders and documenting care.
• Performing client assessments and case management.
• Documenting observations and interventions using electronic medical records on tablets.

Qualifications:

• Current Licensed Practical Nurse license in New Hampshire.
• At least one year of recent, verifiable LPN experience.
• Graduation from an accredited nursing program.
• Prior home care experience preferred but not required.
• Ability to work independently, manage time effectively, and travel as needed.
• Strong interpersonal skills and computer proficiency, with EMR experience preferred.
