BAYADA Home Health Care

is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Concord office team. This office services adult clients on a per visit basis in homes throughout Concord and surrounding towns, NH.

One year of clinical experience as a Licensed Practical Nurse is required. As a home care nurse, you will be an essential part of a multidisciplinary healthcare team providing skilled nursing and rehabilitative care, enabling clients to receive necessary medical care while remaining at home.

Licensed Practical Nurse (LPN) Responsibilities:

1. Making home visits to clients in designated areas.
2. Administering medication, wound care, treatments, and procedures.
3. Monitoring client conditions and reporting changes.
4. Following and documenting doctors' orders accurately.
5. Performing client assessments as needed.
6. Case management and coordination.
7. Documenting observations, interventions, and evaluations using electronic medical records on a tablet.

Qualifications:

• Current LPN license in New Hampshire.
• Minimum of one year recent, verifiable LPN experience.
• Graduation from an accredited nursing program.
• Prior home care experience preferred but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to assigned cases.
