BAYADA Home Health Care

is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in our Concord office. This office services our adult clients on a per visit basis in homes throughout Lebanon and surrounding towns, NH. One year of prior clinical experience as a licensed Practical Nurse is required.

As a home care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, enabling them to receive the medical care needed to remain at home.

Licensed Practical Nurse (LPN) Responsibilities:

• Making home visits to clients in designated areas.
• Administering medication, wound care, treatments, and procedures.
• Monitoring clients' conditions and reporting changes to managers.
• Following doctors' orders and documenting care accurately.
• Performing client assessments as needed.
• Managing cases and coordinating care.
• Documenting observations and interventions using electronic medical records on a tablet.

Qualifications:

• Current Licensed Practical Nurse license in New Hampshire.
• At least one year of recent, verifiable LPN experience.
• Graduation from an accredited nursing program.
• Prior home care experience is preferred but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and computer proficiency, with EMR experience preferred.
• Ability to travel to assigned cases.
