As a Home Health LPN, You Will

1. Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirement, healthcare standards, laws, regulations, and CenterWell Home Health policies.
2. Provide skilled nursing interventions in treatment, rehabilitation, and preventive care, utilizing a holistic approach and therapeutic interventions according to the plan of care.
3. Apply clinical knowledge and skills to facilitate problem resolution and achieve patient-specific goals.
4. Consult with the RN Supervisor regarding changes in the Plan of Treatment and communicate orders from physicians as permitted by law.
5. Implement the nursing plan of care using a goal/outcome-driven approach.
6. Maintain contact with patients, physicians, and healthcare team members regarding patient needs and status changes, participating in care coordination and discharge planning as appropriate.
7. Uphold confidentiality and share information only when appropriate and in the best interest of the patient.
8. Communicate effectively to ensure the availability of supplies and equipment needed for safe care.
9. Document visits, care activities, and notes as required by the company and/or facility.

Required Experience/Skills

1. Graduate of an accredited LPN/VN program.
2. Current nursing license in the practicing state.
3. Valid driver’s license, auto insurance, and reliable transportation.
4. Current CPR certification.
5. At least two years of clinical experience as an LPN/LVN, preferably in home health or hospice.

Pay Range

• $28.00 - $40.00 per visit/unit
• $44,600 - $61,400 per year (base pay)
