Qualifications

• Must hold a valid LPN license to practice nursing.

Responsibilities

• Schedules participants for medical appointments.
• Performs nursing assessments to identify needs and problems.
• Enters data into medical records and clinical logs.
• Provides wound care in clinic and in participant homes per orders.
• Administers medications and conducts EKG tests.
• Makes home visits as required and coordinates with the interdisciplinary team.
