Job summary

Automatic Data Processing, Inc. is seeking a licensed practical nurse to join our team, providing nursing care in clinic and home environments for frail elderly participants under supervision of providers.

The role requires Virginia LPN licensure, a valid driver's license, and at least one year of experience. Duties include assessments, treatments, immunizations, and coordinating care with other health professionals.

Qualifications

• Graduate of an accredited nursing program and Virginia LPN license.
• Minimum of one year’s experience with frail or elderly populations.
• Valid Virginia driver’s license and ability to pass a criminal background check.
• Compliant with state/federal direct patient care requirements; up-to-date immunizations.
• Medically cleared of communicable diseases.

Responsibilities

• Assess participants' health needs and contribute to care plans.
• Provide treatments, medication administration, and preventive care.
• Coordinate patient care with providers and other health staff.
• Ensure HIPAA compliance and maintain confidentiality.
• Participate in on-call coverage and staff training.
• Perform nursing assessments and document findings.
