Job summary

Appalachian Agency For Senior Citizens Inc. is seeking a Licensed Practical Nurse to participate in assessment, planning, and delivery of nursing care for elderly participants in clinic and home settings.

You will work under supervision of the nurse practitioner, physician assistant, RN and other providers to implement care plans. Responsibilities include nursing assessments, administering treatments and medications, immunization coordination, and teaching families about managing health

Qualifications

• Graduate of accredited nursing program with Virginia LPN license.
• Minimum 1 year experience with frail/elderly population.
• Must pass criminal background check and be medically cleared.

Responsibilities

• Perform and document nursing assessments for participants.
• Provide treatments, medication administration, diagnostic and preventive measures.
• Immunization screening and ensure vaccinations are up to date.
• Teach participants and caregivers regarding self-help in health problems.
• Coordinate patient care with other health care providers.
• Maintain HIPAA confidentiality and report suspected abuse or neglect.
