Job summary

Northshore Home Health Care is seeking an LPN to provide patient care using the nursing process under supervision in a home health setting. You will coordinate care with multidisciplinary teams and ensure patient rights and confidentiality are maintained.

The role requires lifting up to 50 lbs and the ability to stand, walk, and reach during shifts, with compliance to State Board standards and ongoing certification as needed.

Qualifications

• Graduate of an accredited practical nursing program or equivalent experience
• Valid LPN license and compliance with State Board of Nursing standards
• Preferred certifications include BLS, ACLS, PALS, PEARS, S.T.A.B.L.E., and LPN-IV, with ongoing education as needed
