Licensed Practical Nurse (LPN)

Mount Carmel Care Center is a faith‑based, non‑profit long‑term care facility located in the Berkshires.

We are seeking an LPN to work all shifts. Compensation ranges from $35.50 to $40.50 per hour based on experience, with additional shift differential pay. A $1,200 sign‑on bonus is offered for full‑time candidates.

Responsibilities

• Assist the Unit/Shift Supervisor in planning, organizing, developing, and directing nursing unit activities.
• Assist the Unit/Shift Supervisor with problem identification and the development of correction plans.
• Check all ill residents early in the shift, perform periodic assessments, and report changes.
• Assess residents’ care needs for Minimum Data Set, Resident Assessment Protocols, and Interdisciplinary Care Plans.
• Report resident condition changes and document them in the medical record.
• Administer and record prescribed medications and treatments.

Qualifications

• Graduate of an accredited nursing program.
• Licensed Practical Nurse (LPN) license in Massachusetts.
• BLS certification.
• Preferred: 1 year nursing home experience.
• Ability to supervise and delegate tasks.
