Job summary

Amedisys Home Health in Maryland is seeking a Licensed Practical Nurse to provide high-quality in-home patient care on a full-time basis in Central and Western AA County. The role offers competitive pay and comprehensive benefits for eligible employees.

Responsibilities include verifying plans of care with the RN, collaborating with physicians, teaching patients and caregivers, taking vitals, reconciling medications, and performing designated procedures with aseptic technique.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit; perform specialized procedures ordered by physician (e.g. Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
