Lisle Creek Estates is seeking a Licensed Practical Nurse (LPN) to join our assisted living team in Lisle, IL. The role provides licensed nursing services under Senior Lifestyle Corporation guidelines, with a focus on safety, accuracy, and compassionate care.

The LPN will manage medication administration, participate in admissions, develop care plans, monitor health status, and educate residents and staff. Flexibility for shifts and a commitment to quality care are essential.

Qualifications

• Must hold a Licensed Practical Nurse license current in the practicing state.
• CPR certification required.
• 1+ year experience in senior care or related field.
• English proficiency to perform duties and respond to emergencies.

Responsibilities

• Administer medications in accordance with Federal & State regulations and guidelines.
• Assist with new resident admissions, including assessments and documentation.
• Develop or contribute to acute and long-term care plans or service plans.
• Monitor clinical indicators (e.g., wounds, infections) as directed.
• Provide health-related consultations and notify physicians/family of changes.
• Respond quickly to emergencies and coordinate with staff.
• Educate residents and staff on health topics; maintain confidentiality.
• Assist with discharges, assessments, and clinical documentation as needed.
