Overview: Licensed Practical Nurse, Assertive Community Treatment Team

Heritage has full time community based LPN positions that work with our Assertive Community Treatment (ACT) team focused on providing services to individuals with severe and persistent mental illness or serious mental illness. This ACT team is a multidisciplinary team working with individuals with severe and persistent mental illness and dual disorders living in the community. This position will provide services in an array of settings including but not limited to, an individual’s home, hospital, emergency room, medical providers or other social service agencies. This program assists individuals in maintaining optimal wellness in the community. Identify, assess, and treat health needs as they arise.

Core Responsibilities Include:

  • Engage person served in their care in community settings (home, nursing facilities, public places, agency, group homes, healthcare organizations, et cetera).
  • Identifies key health indicators to monitor person served overall wellness and healthcare needs.
  • Collects, assesses, and monitors vital checks including blood pressure, blood sugar, and weight at established timeframes
  • Responsible for ensuring person served referral, linkage, and attendance to annual medical evaluation visits, specialty health care visits, and any other health-related activity
  • Consultation and collaboration with all healthcare providers to ensure sharing of information amongst all providers
  • Request and confirm receipt of all evaluations, labs, testing, and healthcare, to allow for follow up treatment
  • Communicate effectively with other providers, staff, and person served.
  • Participate in treatment team meetings with shared providers to collaborate on how to assist person served (s) in their recovery
  • Individual and group interventions to provide training, education, and support to those with behavioral health, health conditions, and/or to help prevent health conditions
  • Individual and group activities including physical exercise such as walking, yoga, smoking cessation healthy living, diabetes self-management, and other wellness activities
  • Completion of all documentation and tracking in a timely manner
  • Provide other duties as assigned, required or specifically negotiated with supervisor.

Knowledge, Skills and Abilities:

  • Ability to maintain confidentiality at all times according to the Illinois Mental Health Code and Confidentiality Act.
  • Demonstrate flexibility and adaptability with changes to program needs and schedule as the need arises.
  • Interactions with individuals and fellow staff members are supportive and respectful
  • Excellent problem solving and decision-making skills

Education and Experience:

  • High school diploma or equivalent
  • Active and undisciplined Licensed Professional Nurse in the State of Illinois
  • Previous experience in behavioral health or human services preferred.

Salary Range: $54,080-$72,800 per year ($26 - $35 per hour) dependent on education and experience
